- All completed and signed medical forms: full completed physicals, health history update forms, asthma/diabetes/allergy plans, pre-filled out physical exam packets (needed in order to sign up for a free school physical ) must be submitted to the health office NO LATER than 6/8/2023
- Online Activity Registration must be complete by July 15th, 2023

- All athletes must be medically cleared and registered for their activity via the online registration portal in order to receive final clearance status

Valid examination must have been conducted 365 days before the first day of the athlete’s official practice.
*IF NO, it was not, THEN YOU WILL NEED A FULL PRE- PARTICIPATION PHYSICAL EXAM
*IF YES, it was, THEN YOU WILL ONLY NEED A HEALTH HISTORY UPDATE FORM
FORMS CAN BE PRINTED OFF THE MOHS ATHLETICS HOMEPAGE OR PICKED UP IN THE ATHLETICS OFFICE
ADDITIONAL MEDICAL FORMS, IF APPLICABLE:
**If you have Asthma, a life-threatening allergy or other pre-existing condition, you will need to update the MD orders for the 2022-23 school year.**
**Students will not be eligible to practice without current Doctor’s orders.**

Once you complete these steps, your coaches and/or advisors will be informed if you have been cleared to participate. You will also receive a final clearance email from the athletic department. Finally, you can check your status by logging in to your family account.
