UPDATED ATHLETIC REGISTRATION AND PHYSICAL PROCESS
Welcome to Mount Olive High School Athletic Registration and Medical Clearance Procedures-
Dear Mount Olive Families,
Student-athletes must complete a yearly physical exam to participate in school athletics and submit it to the school nurse for clearance.
Students are encouraged to seek physicals from their trusted healthcare providers. Involving a student's primary healthcare provider ensures consistent care, allowing for ongoing medical attention and personalized care based on their medical history.
While physicals may also be obtained from our district doctors, these physicals will now take place at Skylands Orthopaedics in Hackettstown instead of our school grounds. This location change is necessary to provide the most appropriate environment for medical exams and accommodate the growing number of students seeking physicals through the school doctors. The Skylands Orthopedic offices will be free of charge to those who do not have access to medical insurance through the district. Those with health insurance must utilize their health benefits to fund the physical. Please get in touch with Skylands directly (908-813-9700) to schedule your child’s physical. It is highly recommended that families call as soon as possible to schedule an appointment to ensure there is sufficient time to secure a physical.
Please note:

Once the physician completes your physical, please bring the form signed by your healthcare provider to your school nurse. The school nurse will review the form and send it to our MOTSD school doctor, who will provide final administrative clearance.

Make doctor appointments early to meet deadlines for athletic clearances. Physical forms should be hand-delivered to the school nurse.

Sport In-Take Days
The nurses’ office will be accepting sports forms throughout the school day. Please remember that students must report to class first and come down with a pass. Teachers will call to let the nurses know a student is coming.
We appreciate your understanding that the nurses’ office prioritizes students with medical needs. If you’re asked to try again later, we kindly ask that you do so.
We also ask that parents refrain from calling to check on the timing for forms or visits, as this can slow down the process. Thank you for your cooperation!

MEDICAL FORMS AND EVALUATION:
Any physical exam performed after July 1, 2024, must be completed on a new PPE packet as per NJDOE and NJSIAA. For your convenience, the new packet is attached below. Please note that the physical exam will now be housed with the physician who performed it and will no longer be held within the school's health office.
All students who participate in the athletic program are required to obtain a physical examination as per NJ state law. A physical is required every 365 days. Each student whose physical examination was completed more than 90 days before the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian.

*If you have asthma, a life-threatening allergy, or other pre-existing conditions, you must update the MD orders for the school year. *Students cannot practice without a doctor’s current orders.*

Once you complete these steps, your coaches and/or advisors will be informed if you have been cleared to participate. You will also receive a final clearance email from the athletic department.

CRITERIA TO PARTICIPATE:

The School Nurse must receive all signed medical forms (NO ELECTRONIC SUBMISSIONS WILL BE ACCEPTED).

Hand your Medical Eligibility or Health History Update forms directly to the school nurse.
To PARTICIPATE, all athletes must:
Have completed and doctor-approved Annual or Physical Update Form
Hand-delivered your Medical Eligibility or Health History Update forms to the nurse.
Registered for their sport via the Online Registration Portal
Received final clearance status from our school doctor and athletics department.

ADDITIONAL NOTES/RESOURCES:
- Paperwork takes up to seven days to be processed for Athletic clearance.
- Please remember to submit any medical plans annually.
VIII. COACHES CONTACT INFORMATION:
Once you complete these steps, your coaches and advisors will be informed if you have been cleared to participate. You will also receive a final clearance email from the athletic department. Finally, you can check your status by logging in to your family account.
