UPDATED ATHLETIC REGISTRATION AND PHYSICAL PROCESS

Welcome to Mount Olive High School Athletic Registration & Medical Clearance Procedures-

SPRING 23-24

 

 

Dear Mount Olive Families,

Student-athletes must have a physical exam completed each year for participation in school athletics and submitted to the school nurse for clearance. 

Students are encouraged to seek physicals from their trusted healthcare providers. Consistent care is ensured by involving a student's primary healthcare provider, allowing for ongoing medical attention and personalized care based on their medical history. 

While physicals may also be obtained from our district doctors, these physicals will now take place at Skylands Orthopaedics in Hackettstown instead of our school grounds. This location change is necessary to provide the most appropriate environment for medical exams and to accommodate the growing number of students who are seeking physicals through the school doctors. The Skylands Orthopedic offices will be free of charge through the district to those who do not have access to medical insurance. Those with health insurance must utilize their health benefits to fund the physical. Please contact Skylands directly (908-813-9700) to schedule your child’s physical. It is highly recommended that families call as soon as possible to schedule an appointment to ensure there is sufficient time to secure a physical. 

Please note:  

  • Once the physician completes your physical, please bring it to your school nurse.  The school nurse will review the paperwork and send it to our MOTSD school doctor, who will provide final clearance. 

  • Make doctor appointments early to meet deadlines for athletic clearances. Physical forms are due to the school nurse by Monday, October 23rd.  

  • Physical forms may be completed from other facilities if you wait to see your doctor.

  1. THE FIRST OFFICIAL DAY OF SPRING SPORTS:

  • BASEBALL and SOFTBALL P/NP - MARCH 14

  • GOLF (SECTIONALS/IND) MARCH 14

  • B/G LACROSSE - MARCH 14

  • TENNIS (TEAM/IND) - MARCH 11

  • TRACK (SECTIONALS/MOC) - MARCH 14

  • BVOLLEYBALL and FLAG FOOTBALL - MARCH 14

 

  1. SPECIFIC SPORT IN-TAKE DAY:

Each athlete must deliver their paperwork to the nurse on the assigned day.  

2:30 pm - 5:00 pm @ School Nurse Office

  • Tuesday 1/30 - Track and Field and Golf
  • Thursday 2/1 - BVolleyball and Baseball
  • Wednesday 2/7 - Softball and Flag Football
  • Wednesday 2/21 - Boy's Lacrosse and Tennis
  • Tuesday 2/27 - Girl's Lacrosse and Unified Sports

 

  1. MEDICAL FORMS AND EVALUATION:

 

All students who participate in the athletic program are required to obtain a physical examination as per NJ state law. A new physical is required every 365 days:

*IF YOU HAVEN’T HAD ONE: THEN YOU WILL NEED A FULL Full Physical Annual Exam

*IF YOU HAVE HAD ONE, THEN YOU WILL ONLY NEED A Health History Update Form

Forms can be picked up in the Athletic Office or printed online.

 

To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days before the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian. 

 

  1. ADDITIONAL MEDICAL FORMS, IF APPLICABLE:

*If you have asthma, a life-threatening allergy, or other pre-existing conditions, you must update the MD orders for the school year.  *Students cannot practice without a doctor’s current orders.*

Once you complete these steps, your coaches and/or advisors will be informed if you have been cleared to participate. You will also receive a final clearance email from the athletic department.

 

  1.  CRITERIA TO PARTICIPATE: 

The MOHS School Nurse must receive all signed medical forms (NO ELECTRONIC SUBMISSIONS WILL BE ACCEPTED):Hand your forms in on your assigned In-Take day.  All submissions must be handed in no later than 3/1/2024

 

 

To PARTICIPATE: All athletes must:

1. Have completed and doctor-approved Annual or Physical Update Form

2. Attended an In-Take Day

3. Registered for their sport via the Online Registration Portal

4. Received final clearance status from our school doctor

 

 

  1. ADDITIONAL NOTES/RESOURCES:

Once the physician completes your physical, please bring it to your school nurse DURING your assigned In-Take Day. The school nurse will review the paperwork and send it to our MOTSD school doctor, who will provide final clearance.

  • If your family does not have healthcare insurance, you can make an appointment with our school affiliate, Skylands Orthopaedics in Hackettstown, by appointment only.
